The CARE Project

IEP, IFSP, 504 plan

The CARE Project (Creating an Albinism Resource for Education) provides parents of school-aged children with albinism, educators and other professionals with resources and information to assist them in formulating a child with albinism’s education document. Education documents include Individual Family Service Plans (IFSPs)Individual Education Plan (IEPs) and / or “504 Plans,” i.e. plans pursuant Section 504 of the Rehabilitation Act of 1973.

You can access the CARE Project database to review actual IFSPs, IEPs and / or 504 Plans for children with albinism for help on myriad topics such as grade level goals, assistive technology recommendations, orientation and mobility goals, age objectives and social development objectives. For more information, please contact the CARE Team.

In addition to the complete education documents, the CARE Team (all NOAH volunteers) create and maintain a database of selected information, i.e. goals, objectives, accommodations, assistive technologies, transition items, etc., extracted from the education documents. This information is presented in spreadsheet formats that are available for viewing and /or download at the following links. Please note that creating the database spreadsheet is a manual process, and thus there may be typos. Please let the CARE Team know if you find them.

*** PLEASE NOTE: All children with albinism have different visual acuities, different functional vision levels and different personal developmental needs. The purpose of the CARE Project is to inform parents and professionals of the “spectrum” of goals, technology and educational methodologies implemented in our schools to assist children with albinism. The materials are intended to provide parents and professionals with a broad overview to better aid them in establishing a child’s individual and unique education plan.

How to Submit Education Document Examples

The CARE Project is stronger with YOU!  Please consider submitting your child’s education documents, including IFSPs, IEPs and 504 Plans.  To submit documents, please:

  1. Redact as much personally identifying information as possible before submission. * The CARE Team will also review the documents to ensure that all personally identifying information is removed.
  2. Send in the education documents and accompanying submission forms via one of the following methods: (a) emailing them to the CARE Team, or (b) mailing them via postal mail to: NOAH CARE Project, PO Box 959, East Hampstead, NH 03826-0959. With your submission, please include the following information:

* Upon receipt, the CARE Team will remove all personal and other identifying information from your documents, i.e. “scrub” them, and then extract selected information for the CARE database. They will then send the scrubbed documents to you for your final review and comments, and thereafter will add the final version to the databases.
